Beckley, West Virginia Man Injured at Sea:

On April 18, 1996, while employed as a crewmember aboard a fishing vessel, a Beckley, West Virginia man was seated in a revolving chair in the wheelhouse that had a broken locking mechanism.  The boat took a hard roll and the broken locking mechanism failed.  The chair spun and threw him out, head first, right down a stairway to the deck below where he landed on his head and suffered a broken neck.
